Our System of Care

We believe all children have the right to grow up safe, healthy and fulfilled with families who love and nurture them.

That’s why our system of care is family-centered, strength-based and community-driven. It facilitates a single point-of-entry that brings consistency to children and families as they enter the system and provides the team with a standard assessment of the child. That assessment enables the care manager to meet the child’s needs and make an initial placement that provides the stability and security necessary for the child to cope with the crisis they are facing, with minimal emotional damage.

Our system of care includes the following services to most effectively meet the needs of the children and families we serve: 

Mobile Response

Teams are mobilized to assist in crises and stabilize the family in need, if appropriate. This is a 24/7 service that helps ensure children’s safety and stability in their homes.

Family Team Conferencing

Composed of family members, friends, care managers and others in the community who may have a relationship with the family (teachers, clergy, coaches), this team is created to provide the family with the necessary support to ensure the success of their care plan. It also provides the family with a voice and ownership in their plan. Data indicates that this element is a pivotal factor in the success of the families utilizing our services.

Flexible Supports

Plans adjust to the needs of the family. One plan does not fit all families, nor does one method fit one family at all times. Many families experience crises at different times in their lives, requiring the need to reassess services provided.

Wraparound

We build a network of support around a family. The wraparound philosophy ensures the family’s needs are met with the appropriate services at the proper time.
