Service Provider Interest Form

Family Partnerships of Central Florida currently contracts with both service delivery providers and placement providers who meet identified service gaps within our four-county service area: Orange, Osceola, Seminole, and Brevard Counties.

We are actively seeking providers who:

• Deliver in-person services within their office or the client home

• Can serve incarcerated individuals within the correctional facility

• Provide psychological evaluations in Spanish and/or Haitian Creole

• Offer transportation services to support clients

Submission of an interest form does not guarantee a contract opportunity. All submissions are reviewed based on current service needs, geographic coverage, and identified system gaps.
